We may collect data directly from you via telephone conversations, email correspondence, our website online booking form, our clinic consent form, face to face during a consultation, or form your feedback.
We may also collect data from a family member, or someone else acting on your behalf.
Type of personal Information collected
In order to provide you with the very best care we will ask for your name, date of birth, address, email address and your GP details. We will also collect information about your ear symptoms, medical history, conditions, current medications and allergies. We may collect medical imaging of your ear to enhance your consultation experience and understanding of your ear health and care and where necessary for onward referral (such as to your GP).
How will we store your data?
Your electronic and paper records will be securely stored and only accessed by Hear Again Exmouth. In line with government guidelines, we will retain your health records for eight years. They will then be reviewed prior to destruction taking into account any serious incident retentions.
Sharing your personal information
We will only share you information with your consent where there is a clinical need, such as a referral to your GP. We will not disclose any health information to third parties without your consent, unless there are circumstances such as when the health or safety of others are at risk or where current legislation permits or requires it.
